- If the name under the age of 21 is to change the name, the statement should be forwarded by the parent / guardian of their respective children.
- It is recommended that the new name be used for at least one year.
Children below the age of 7 years
- Original birth certificate to be amended
- Parent certificate of marriage / Birth certificate
- Mother's / father's National Identity Card
- Affidavit that there is no objection to father and mother's protest.
- Recommendation from Grama Niladhari confirming amendment
- Gene recommendation.
- This declaration should be handed over by the child's parent / guardian.
- Please come only on Mondays or Wednesdays to make the statement public.
Correction of a mistake that occurred in the mother's area of birth certificate

- If the death is less than 1 year, if the stamp is more than Rs. 50 / sterling over 1 stamp
- Birth certificate of the publisher / marriage certificate and a copy of the National Identity Card
